In order to apply to be a worker with Pro-Health Care, bring the following items with you to the office:

Green Card or Work Permit (if not a US citizen)

Bank routing and account numbers (to set up direct deposit)

$40 Refundable Application Fee
For PCA/CFSS worker position:

Copy of DHS PCA/CFSS Training Certificate (if you have it)
For HHA/Waiver worker position:

TB Blood Test or X-ray Results
