We are always looking for dedicated individuals to join our broad team of home health care staff.

In order to apply to be a worker with Pro-Health Care, bring the following items with you to the office:

Driver's license, state ID, or passport

Social security card
-
Green card or work permit (if not US citizen)
-
DHS CFSS Training Certificate (for PCA/CFSS, if you have it)
-
TB blood test results or X-ray results (for HHA only)
-
Bank routing and account numbers (to set up direct deposit)

$40 refundable application fee
